1. HRT plan 1 - $75/mo labs ordered, up to quarterly appts in-person or Tele, & Rx sent to pharmacy.
2. HRT plan 2 - $100/mo all of above plus option for Bio-Identical HRT Pellets Females every 3 months Men every 6 months.
3. HRT plan 3 = $125/mo all of above plus option for Bio=identical HRT Pellets Females every 2 - 2.5 months. Men every 4 - 5.5 months.
Family Primary Care Option
Are you covered by a Health Sharing Plan? Are you paying for every office visit? We can help.
For a family of 5 or less $25/mo, 10 or less $50/mo, more than 10 $75/mo.
This gets you a sick visit if needed for every member of your family each month, a quarterly appt and the prescriptions submitted. If more appts needed it is at a reduced cost of $74 per appt reduced from $99.
